Mission
The mission of The Nib & Betsy Scheben Intendance Heart Developed Twenty-four hour period Health Program is to provide a daytime program of supervised therapeutic action, supportive health care and support for the attainment of personal goals to medically compromised and/or disabled younger adults and the elderly, thereby improving their quality of life and delaying or eliminating the demand for institutionalization. Our goal is to empower the disabled and elderly to cocky-advocate, pursue personal growth and independence and have choices in every twenty-four hours life.

Adult Day Health Care
This medical model programme services persons over the age of 16 years of age who have medical, concrete, social, and/or cerebral disabilities. The program is run by an RN and staffing includes LPN, C.N.A.'s and nursing assistants.
The major goals of the programme is to provide nursing care and supervised activities based on rehabilitative principles to medically compromised and/or disabled adults in order to maintain their ability to function in the domicile and community, to provide respite for families/caregivers in guild to reduce the physical, emotional and fiscal stress experienced during long-term care; and to preclude unnecessary or premature institutionalization of persons appropriate for intermediate level of nursing domicile care.

What is the system aiming to accomplish?
To provide nursing intendance and supervised activities to medically compromised and/or disabled adults and the elderly. To provide respite intendance for families/caregivers in club to reduce the physical, emotional and financial stress experienced in long-term care. To reduce the incidents of physical corruption and/or neglect. To prevent unnecessary or premature institutionalization or comprisal in to a long-term intendance facility.
What are the organization's fundamental strategies for making this happen?
This is done by providing an individualized plan of care which includes a health and functional assessment, identifying the person's strengths and needs, specific goals are developed to run into those needs and activities to accomplish those goals.
What are the organization'south capabilities for doing this?
Our capabilities are to provide individualized medical and nursing services, nutritional services, rehabilitative services, educational and socialization services. Our squad is comprised of registered nurse, licensed practical nurses, certified nursing administration and activity coordinator. Physical therapists, occupational therapists, speech therapists and psychologists are consulted as needed.

Progress is measured by daily/monthly progress notes towards goals. Reassessments are done equally needed, but at least annually to update needs/goals.
